The game looks like it’s tricking you but actually isn’t. There are always two witches and they’re both real, you just don’t get to see one of the health bars at first. You can still damage the one with the hidden health bar
As the other commenter said, the other witch is just invisible at first, but you damage both in phase 1. If you have at least 1 insight, these lanky dark figures with long claws will start to appear:
So ideally, you want to not have any insight at all going in to make the fight easier and need to keep an eye out for the second witch to not get ambushed
