Birth control pills are a common form of contraception. The pill bottle in the picture insinuates that the subject will use them, using the water to help swallow.
Sure, though birth control pill containers don’t look at all like regular pill bottles.
Based on the original version of this meme, OP has almost certainly never taken birth control, also it’s funny and I don’t think it’s an unreasonable reuse of the original image.
(In before someone else mentions being prescribed some weird uncommon BC that came in a standard pharmacy pill bottle, because I’m sure that’s happened to someone at some point in history.)

The photo doesnt have a specific meaning, the text came first and the image was made to fit rather than other way round. Probably the source of that disconnected feeling.
